Direct payment
AXA Assistance can help arrange direct billing (Direktabrechnung) with hospitals abroad; otherwise the insured pays and submits original invoices, which are reimbursed by AXA, with foreign-currency costs converted at the ECB reference rate on the day the documents are received.

Medically necessary repatriation

Additional costs of a medically sensible and justifiable repatriation to the nearest suitable hospital at the insured's home are reimbursed at 100% if organized by AXA/AXA Assistance or pre-approved by AXA. If only medical sensibility (e.g. treatment abroad expected to exceed 14 days, or treatment costs abroad exceeding repatriation costs) is established without such organization/pre-approval, reimbursement is capped at EUR 5,000 within Europe and EUR 10,000 outside Europe.
Repatriation of remains

In the event of death of the insured during the trip, direct costs of repatriation to the home address at the start of the contract are reimbursed up to EUR 5,000 from within Europe and up to EUR 10,000 from the rest of the world; burial costs abroad up to the same limits may be paid instead of repatriation. All repatriations must be coordinated with the insurer.
Pre-existing conditions

There is no blanket exclusion of pre-existing conditions. Excluded is treatment of illnesses/injuries for which the trip was undertaken, and treatment that was already foreseeable as necessary during the planned course of the trip at the time of departure (unless the trip was made due to the death of a spouse or first-degree relative).
Pregnancy complications

Investigation and treatment for pregnancy, childbirth, miscarriage and termination (and their consequences) is generally excluded, but cost reimbursement is provided to the extent medical assistance in the country of stay is necessary for acute pregnancy complications, including miscarriage.

Key conditions
Assistance call deadline
Any hospital treatment must be reported to AXA within 10 days of its start; all supporting documents/receipts for a claim must be submitted no later than the end of the third month after the trip ends.
Unauthorized treatment
If the policyholder violates the reporting/cooperation obligations, AXA may be released entirely from the payment obligation in case of intentional breach, or may proportionally reduce benefits in case of gross negligence, in accordance with §10 AVB-R in conjunction with §28 VVG.
Purchase while abroad
The policy must be taken out before the trip begins and before crossing the border abroad; trips where departure from Germany occurred before the insurance start date are excluded from cover, and the policy cannot be extended retroactively while already travelling.
Chronic conditions
Chronic conditions are not explicitly excluded as such, but treatment is excluded if the trip was undertaken for the purpose of that treatment, or if the need for treatment was already foreseeable and scheduled to occur during the planned course of the trip.
Pregnancy
