Direct payment
Envivas operates primarily on a reimbursement basis: policyholders pay providers directly and submit invoices for reimbursement. The exception is inpatient hospital stays, for which Envivas provides a guaranteed direct cost-assumption commitment to doctors and hospitals.

Unauthorized treatment
